If Construction:
( ) Construction budget (sign by professionals)
( ) Construction Surveys (municipality)
( ) Chronogram of bank payments against const. advance (sign by professional)
PLEASE FURNISH THE FOLLOWING IF SELF EMPLOYED, OR COMMISSION OR BONUS INCOME EXCEEDS 25% OF GROSS INCOME (in addition to the above 3 items)
