This checklist has been provided to help you gather information to include with your loan application.

( ) Uniform Residential Loan Application, completed and signed by all parties

( ) W-2 forms for previous 2 years (bring original W-2 forms)

( ) Personal tax returns (all pages) for prior 2 years (signed and dated)

( ) Original Bank statements and/or brokerage account statements for prior 6 months

( ) Earnest Money Contract (executed by Buyer and Seller)

( ) New Know your customer formula (attached)

( ) Buy contract

( ) Sugef Letter

( ) ID Copy

If Condominium:

( ) Condominium Act.

( ) Certification of Condominium administration payment

If Construction:

( ) Construction budget (sign by professionals)

( ) Construction Surveys (municipality)

( ) Chronogram of bank payments against const. advance (sign by professional)

PLEASE FURNISH THE FOLLOWING IF SELF EMPLOYED, OR COMMISSION OR BONUS INCOME EXCEEDS 25% OF GROSS INCOME (in addition to the above 3 items)

( ) Corporate/Partnership tax returns with all schedules attached for prior 2 years

(signed and dated)

( ) Profit and Loss Statement for year to date (signed and dated)

( ) Balance Sheet for year to date (signed and dated)
